After Care Case Management

| The ILSP After Care Program, Beyond Emancipation, supports and assists recently emancipated youth (to age 24) as they make the transition to independent living. The Beyond Emancipation staff conducts assessments regarding mental and physical health issues, housing, employment, transportation, and education. Following the assessment, the youth works with theBE Case Manage to ensure that he or she has a plan for achieving self-sufficiency using the Transitional Independent Living Plan (TILP). Referrals will be made as needed to EPSDT service providers for intense, one-on-one therapeutic and life skills case management. The BE Staff works closely with the THP+ Programs, CYC Adult Sponsor, Teen Health Clinic staff, Pivotal Point Youth Services, and other CBOs to coordinate delivery of services. |
